New Patients: If you're new to Periodontal Associates you can complete the new patient intake forms before you visit us.

Print and fill out all new patient forms. If you're unable to print and complete these forms, please arrive 10 minutes early for your scheduled appoinment to complete theme in office. Request an appoinment with us online or call our office! A front desk associate will get back to you as soon as possible to ensure proper schedulling. Upon arrival, please bring your complete forms, dental insurance card, Photo ID and referrals or x-rays your dentist has given you.

Dr. Strauss and his staff could not be more professional. They take the time to talk to you and explain what will be done. I am constantly surprised at how little pain is associated with the procedures they perform. My gum issues are completely under control under their care.

I have been a client at this practice for more than 30 years. I have always been treated with professionalism and received great customer care. I am moving out of state soon and certainly hope to find a practice of this caliber!
